Door to Door Home Care (DDHC) was registered in 2017 company striving to empower communities and individuals to take control of their own health. This is possible through improved health literacy and routine health checks.
Door2Door home care has designed health promotion interventions to improve health literacy and early detection at family level by engaging community members in their homes and during community health days.
We shall use local evidence based interventions which are intended to empower community members to take control of the own health; reduce preventable disease, and improved house hold income leading to sustainable development. This will be achieved in partnership with Ministry of Health (MOH), Districts, hospital, PNFPs, NGOs, schools and community leaders.

Home Based
Care
We Offer Bedside Nursing for Chronically ill and Bedridden Clients (Diabetic, Hypertensive, HIV, Cancer, Fractured, etc.). Care for Elderly, Palliative Care, Individualized Treatment Plan at Home, We also do Postnatal Care for Mothers, Adherence Reminders and Counseling for HIV, etc.
Community Health
Interventions
Our Community Outreach Program is a service we offer intended to the benefit of the local community overall. Community outreach services is both physical and can be completed remotely. (Examples: We do sensitization on Health, Nutrition, Free Medical Testing and Screening Services, etc.)
School Health
Promotion
Our School Health Promotion Program is a service we offer intended to the benefit of the local schools community overall. School Health promotion serves is both primary and secondary school on; Menstrual Hygiene Management, Teen pregnancy prevention, Life skills, Health and safety and Psychotherapy.
Training Home Care
Assistants
Under this Service; We offer Training for Training Home care Assistants, teaching them how to care for the sick and training them on hygiene, customer care, compassion and on how to deal with people who need care, Additionally we train them on Physiotherapy.
